The Ontario government is developing a connected healthcare system that prioritizes patients, families, and caregivers by strengthening local services and improving coordination between providers. Ontario Health Teams, which are being established across the province, aim to provide integrated care that is more accessible and patient-centered.

There are 58 Ontario Health Teams, ensuring full provincial coverage. These teams bring together a variety of healthcare providers—including hospitals, doctors, and home and community care specialists—to deliver coordinated care across different settings. Eligible organizations for Ontario Health Teams include those offering services such as primary care, acute care, mental health support, rehabilitation, palliative care, emergency health services, and more, ensuring comprehensive support for all Ontarians.
