OHTs are becoming the provincial vehicle to implement change in the health care system. These changes ultimately improve the health of the population. In 2024-25, The BOHT has priorities in both improving care for patients (clinical priorities), as well as how our local health system is organized (structural priorities).
Clinical Priorities:
- To support the integration of care by coordinating health and social services in Burlington and surrounding communities.
• Expansion of the Community Remote Care Management Program
• Spread of the Community Wellness Hub model - To make navigating the health care system easier for patients.
• Curate local service information across Health811 and the Burlington Health Navigator App - To assist patients have access to primary care, especially those who don’t have a regular primary care provider.
• Support of the Prime Care Family Health Team Gender Care Clinic
Structural Priorities:
- To ensure the BOHT is prepared to delivery integrated home care services.
• Creation of Integrated Home Care Committee - To incorporate as a not-for-profit organization by through collaboration with members.
• Incorporated as a not-for-profit organization in January 2024. - To engage primary care providers.
• Develop a Primary Care Network and Primary Care Network Leadership Council - Embed a digital and data plan throughout the BOHT
• Partner with Greater Hamilton Health Network and Middlesex London Ontario Health Team to launch HealthPathways in Canada
